RCP is a program where Doctors, Nurse Practitioners, and Physician Assistants provide medical care in the comfort of your home. This way, you don't have to travel to a doctor's office.

RCP offers comprehensive services such as regular health check-ups, managing your medications, and creating personalized care plans to support your health at home. Our Providers can prescribe medications if needed and help coordinate your care after a hospital/skilled nursing facility stay to ensure a smooth recovery.

How does RCP differ from other healthcare programs?
Unlike other programs, RCP brings healthcare directly to your home. You will receive care from experienced medical Providers.

Are telemedicine or virtual appointments available?
Yes, if you prefer or need it, you can have appointments via video call on your phone or on your computer. This allows you to speak with your healthcare provider without having to leave your home.

Can I still see my regular doctor if I join RCP?
Yes, you can continue seeing your regular doctor. RCP works with your doctor to make sure you receive the best possible care.

How do I schedule an in-home visit?
Scheduling a visit is simple. Just call our team, and we will set up a time for one of our healthcare Providers to come to your home.

What happens during a home visit?
During a home visit, a medical Provider will come to your home and obtain your medical history, talk about all your medical concerns, review your medications and conduct a thorough exam similar to what you would experience in a clinic.

How often will I receive visits through RCP?

How do you ensure privacy and confidentiality during visits?

Can family members and/or caregivers be involved in my care plan?
Absolutely! We encourage family members and/or caregivers to be part of your care plan. They can be present during visits; help coordinate your care and talk with us about any concerns or questions regarding your health.

Does my healthcare Provider bring any medical equipment for the home visit?
Yes, our Providers come equipped with essential diagnostic tools such as blood pressure machine, stethoscopes, and other necessary medical equipment.
